A photocopy of one of the following government-issued IDs in French or English: health insurance card, driver’s licence, passport, permanent resident card or asylum seeker document.

An original, recent, passport-style photo of your face: size 2 x 2 inch (5,08 x 5,08 cm).After that time, it is considered expired and can no longer be used. The form is valid for 30 days from the date indicated by the educational institution.For students who will turn 16 by October 31 of the current school year, the form must be stamped and signed by the student’s educational institution.For students aged 15 or less by October 31 of the current school year, the proof of full-time student status is not required.For students aged 13 and under, the form must be signed by a legal guardian.
